jacsm1217: I am guessing that Email addresses and phone numbers are listed in the article for anyone who seriously would like to contribute - they can email for the mailing address. Since the paper can be read on-line, I am guessing that addresses are not listed for privacy reasons. I emailed to express interest in donating and got a very quick response.
