Washington County Grand Jury indicts 41, including Belpre pharmacist

The Washington County Grand Jury indicted 41 people Thursday, including a Belpre Pharmacist for alleged theft of controlled substances, including oxycodone and hydrocodone, from the Fruth Pharmacy in Belpre.

Thomas Andrew Glazier, 55, of 5981 Veto Rd., Belpre, was indicted Thursday on five fourth-degree felony counts of grand theft, one third-degree felony count of aggravated possession of drugs and one fourth-degree felony count of illegal processing of drug documents.

Glazier’s charges stem from actions the indictment alleges he took from Jan. 1 – April 23 to steal hydrocodone, oxycodone, tramadol I (also known as Ultram), lorazepam (also know as Ativan) and alprazolam (also known as Xanax) from the Fruth Pharmacy located at 1401 Washington Blvd. in Belpre, where he was a pharmacist.

The indictment also alleges Glazier knowingly made a false statement that was related to hydrocodone from Jan. 1 to April 23 in a prescription, order, report or record that is required by state law.

A request for a summons was issued by the Washington County Court of Common Pleas on Dec. 12 and was sent to the Washington County Sheriff’s Office, according to court records, and states Glazier must show up to the court at 10 a.m. on Dec. 30 or he may be arrested for failure to appear. Neither Washington County Jail records or court records show that Glazier has been arrested for the charges.

On May 2, the Ohio Board of Pharmacy issue a notice of opportunity for hearing to Glazier based on allegation he was diverting controlled substances from his place of employment, according to the records, and then on Oct. 11 the board issued an order indefinitely suspending Glazier’s pharmacist license for not less than a year from the date of the order.

Glazier “stipulated to the allegations”, the order said, that from Jan. 1 to April 23 he “stole controlled substances from his employer, Fruth Pharmacy #15, located at 1401 Washington Blvd., Belpre, Ohio.”

The order stated Glazier stole approximately 232 hydrocodone 5 mg tablets and approximately 3 hydrocodone 7.5 mg tablets.

On April 23, Glazier admitted to an agent of the Pharmacy Board that he stole approximately two tablets of hydrocodone every shift he worked from January to April 23 in 2024 at Fruth Pharmacy, the order said.

Glazier also said he could potentially be dependent on hydrocodone, the order said.

Glazier cannot work at a facility license by the Pharmacy Board to possess or distribute dangerous drugs and/or medical marijuana during his suspension, according to the order, and after 12 months from the effective date of the board will consider a petition to reinstate Glazier’s license if he meets certain conditions.

This is not the first time the Pharmacy Board has taken actions against Glazier’s pharmacist license.

Glazer entered into a settlement agreement with the board in August 2022, according to board license records, in relation to allegations that he failed to remove his Controlled Substance Ordering System account from his former employer White Oak Pharmacy located at 8465 State Route 339, Vincent. As part of the agreement Glazier agreed to a monetary penalty of $250 and to attend a Responsible Person Roundtable and a written reprimand was placed on his pharmacist license, according to the settlement agreement. Glazier also entered into a settlement agreement with the board in March 2021 in relation to allegations that as the responsible person at White Oak Pharmacy, Glazier allowed a pharmacy employee to perform pharmacy technician duties without them being properly registered with the Pharmacy Board.

As part of the agreement Glazier agreed to a monetary penalty of $250. He did not confirm or deny the allegations in either agreement.

Requests for comment by Fruth Pharmacy Inc. were not returned as of press time.

* Austin William Wright, 34, 401 ½ Marion St., Marietta, one fifth-degree felony count of aggravated possession of drugs, from July.

* Chad Michael Wilson, 38, 2131 Murray St., Belpre, one fifth-degree felony county of aggravated possession of drugs, from May 2023.

* Caleb J. Anglin, 20, 984 Clara Ave., Akron, one third-degree felony county of possession of a fentanyl-related compound and one third-degree felony count of trafficking in a fentanyl-related compound, from July.

* Niaire Danali Clipper, 20, 5257 Fairland Rd., Norton, Ohio, and codefendant D’Anthony Ray Kelker, 19, 1210 N.
